Do I need a referral from a physician to be seen?

Yes. We are now accepting appointments by physician referrals only.

How do I make an appointment?

You can make an appointment with us by physician referral only.

What should I bring to an appointment?

Please bring a photo I.D., proof of insurance, any medications you are currently taking, and a completed new patient form. If you are being seen by physician referral, please bring any medical documentation from another doctor about your area of pain. Which types of insurance do you accept?
